Facelift

?

A facelift (rhytidectomy) is a surgical procedure designed to address the effects of aging in the face and upper neck. As we age, several changes occur that contribute to an older appearance: skin loses elasticity, facial fat shifts downward and the deeper tissues that support facial contours begin to weaken. While a facelift won’t fundamentally change your appearance or stop the aging process, it can help “turn back the clock” and restore a more rejuvenated look.  

Though every patient is different and may require different techniques, Dr. May's preferred technique is the high SMAS deep plane composite facelift, a method he considers the gold standard for facial rejuvenation. This advanced approach lifts not just the skin but the deeper facial structures, including the SMAS (superficial musculoaponeurotic system) and malar fat pads, as a single unit, creating beautiful and natural-looking results.

His advanced techniques avoid the tight or “windswept” appearance common with facelift techniques that focus on just the skin, helping you look refreshed without people wondering “what kind of work” you’ve had done.

Procedure Details

At May Plastic Surgery, a facelift is performed as an outpatient procedure under general anesthesia, taking about two to three hours to complete when done in isolation, though facelifts are commonly performed with 
a neck lift or other facial rejuvenation procedures that will increase 
the surgical duration. While some surgeons may offer local anesthesia 
with sedation, Dr. May prefers general anesthesia for enhanced 
safety and precision.

There are a number of techniques Dr. May can use to rejuvenate your appearance, with the exact approach depending on your needs, goals 
and facial anatomy. The specifics of your procedure will be determined by 
which approach is selected.

Dr. May's preferred technique involves making carefully placed incisions that follow the natural contours of your face, typically beginning in the temporal hairline, curving around the ear and extending into the hairline behind the earlobe. This allows your incisions to blend in seamlessly with the contours of your face, making them inconspicuous and virtually 
invisible once healed.

Rather than simply pulling the skin tight, Dr. May works in the deeper layers of the facial tissues. He releases the retaining ligaments that cause sagging, then tightens and repositions the SMAS layer and facial fat pads to restore a more youthful appearance. To finish the procedure, he will carefully trim and redrape the skin over your newly lifted contours and close the incisions.

What can I expect at my consultation?

The first consultation is a genuine, thoughtful conversation. We want the experience to feel comforting and non-clinical while still meeting all the medical needs of our patients.

We’ll talk in-depth about your goals, concerns and medical history, and I’ll perform a physical exam to help determine the best treatment options. I like to think that my approach is honest and educational. I want patients to leave feeling informed, not pressured. You’ll receive a customized plan and have plenty of time to ask questions. This is your space to explore what’s right for you.

We’ll also make sure to discuss skincare and how to properly prepare for surgery, as well as how to take care of your skin after surgery to ensure the best results possible.

Do you have a preferred facelift technique?

I perform a range of facelift techniques, but my preferred approach is the high SMAS deep plane composite facelift, the technique I consider the gold standard. I trained at UTSW under surgeons who were deeply influenced by Dr. Fritz Barton, a pioneer of this method.

This technique allows for lifting not just the skin, but the deeper facial structures, including the SMAS and malar fat pads, as a composite unit. By working in the deep plane, I can release the key facial retaining ligaments and reposition the tissue in a more natural, tension-free way. The result is a facelift that provides powerful yet elegant rejuvenation with longer-lasting results and less risk of that “pulled” or overly tight appearance.

That said, no two faces are the same. While this is my go-to for comprehensive facial rejuvenation, I also employ techniques like SMASectomy, SMAS plication, or bi-lamellar lifts in select cases, such as revision surgery or when extensive scarring is present. I assess each patient individually to determine the best technique for their anatomy, aging pattern and goals.

How can I be sure my results w ill look natural and not overdone?

Through meticulous planning, customization and execution. I use the latest surgical techniques and maintain a relentless commitment to continuous learning. I also set realistic expectations and follow patients closely during their recovery to ensure optimal results. It is my opinion that some plastic surgeons will simply do the procedure a patient asks for because their friend had it or because they read about it. I offer options and have in-depth conversations about the pros and cons of those options. If a patient’s goals do not align with what I feel is a safe or appropriate treatment, I am more than comfortable telling them no or that I’m not their surgeon.

Is there a “right” age to get a facelift?

A common misconception is that younger patients “don’t need” plastic surgery, or that surgery is only for those well into the aging process. The truth is, everyone ages differently, and in some cases, early or proactive intervention can actually lead to better, more durable results with shorter recovery times. Whether it’s a conservative neck lift in your 40s or early eyelid surgery to address genetics rather than aging, timing is personal, and it’s not about age, but about anatomy, goals and lifestyle.
