What Is Forehead Reduction?
Forehead reduction, also known as hairline-lowering surgery, is a procedure that reduces the size or height of the forehead by advancing the hairline forward. This creates well-balanced facial proportions and can dramatically improve your overall appearance, especially for patients who feel their forehead is disproportionately large compared to the rest of the face.
Dr. May's preferred approach involves a camouflaged hairline (pretrichial incision), strategically placed to bring the forehead down while preserving the hair follicles. As the incision heals, new hair can grow through it, delivering beautiful results without the telltale signs of surgery.

What Can Forehead Reduction Address?
Forehead reduction can create better facial harmony and more balanced proportions. Dr. May's expert techniques can address:
- A large or prominent forehead
- An elongated, unbalanced facial appearance
- Facial feminization when your features don’t match your gender identity
- A high or receding hairline
- Self-consciousness about forehead size
Ideal candidates for this procedure are in good overall health and have realistic expectations. While forehead reduction surgery can greatly enhance the aesthetics of the face, it’s not a guarantee of perfection. This procedure is not typically recommended for individuals with progressive hair loss or conditions like scarring or alopecia that affect the scalp.
The Art & Science of Facial Rejuvenation With Dr. May
There's something uniquely satisfying about solving a complex problem with precision and creativity, a mindset Dr. May developed long before he ever held a scalpel. His undergraduate engineering degree taught him to approach challenges methodically, while his artistic sensibility ensures every solution is as beautiful as it is functional. This blend of analytical thinking and creative vision shapes how Dr. May approaches every facial rejuvenation procedure, seeing the signs of aging not as separate “issues” but as an interconnected system that needs to be rebalanced as a whole.
Technical skill gets you halfway there, but Dr. May’s artistic eye is what separates good results from truly transformative ones, influencing everything from incision placement to tissue repositioning. He also considers factors like natural facial asymmetries, expression patterns and even how someone’s face moves when they smile or laugh to craft each surgical plan.

Procedure Details
At May Plastic Surgery, forehead reduction is performed as an outpatient procedure under general anesthesia. The surgery typically takes one to two hours to complete, depending on the extent of reduction needed, your individual anatomy, and whether your treatment plan includes other facial rejuvenation procedures, such as a brow lift or blepharoplasty.
Dr. May creates a carefully planned incision along the hairline, using a pretrichial incision that advances the hairline while following the contours of your scalp. Through this incision, he removes a precise amount of forehead skin and brings the hair-bearing scalp forward to create better facial proportions. His advanced techniques allow hair to grow through the scar over time, further camouflaging it so the scar blends in seamlessly for discreet, natural results.

Recovery & Results
Recovery from forehead reduction involves initial swelling and mild discomfort that typically peaks within the first few days and then gradually improves. Your scalp will also feel tight. You'll wear a headband for the first week to protect the incision and support proper healing.
Most patients can return to work within two to three weeks, though strenuous activities should be avoided until cleared by Dr. May. The incision along the hairline heals exceptionally well, and as hair grows through the scar, it becomes increasingly difficult to detect.
